What Is a New Starter Employee Health Questionnaire?
Cherries Medical Clinic offers a professional new starter employee health questionnaire service for businesses of all sizes. When you take on a new employee, understanding their health needs from
day one helps you meet your legal duties as an employer, arrange appropriate workplace adjustments, and protect both the employee and your organisation. We can manage the entire
process on your behalf — from supplying and distributing questionnaires through to medically reviewing the responses and issuing clearance letters or full occupational health reports where
follow-up is needed.
Why Use a New Starter Employee Health Questionnaire?
Pre-placement health screening enables employers to identify whether a new employee has any health conditions that may affect their ability to perform the role, and to determine whether
reasonable adjustments are required under the Equality Act 2010. For safety-critical roles, it provides assurance that the employee is fit to start work before any risk arises. More broadly, addressing health needs at the outset reduces the risk of early-onset sickness absence and demonstrates compliance with occupational health duties. For roles that carry health surveillance obligations — such as those involving noise exposure, chemical hazards, or safety-critical equipment — a baseline health assessment is particularly important.
What Our Service Includes
We supply a professionally drafted, legally compliant new starter employee health questionnaire tailored to your sector and role requirements. Completed questionnaires are reviewed medically bycour Occupational Health Physician. For straightforward cases, a written clearance letter is typically returned to the employer within 48 hours. For complex or flagged cases, a full occupational health management report can be produced following a face-to-face or video consultation. The report will include recommendations on reasonable adjustments, phased start arrangements, or further assessment where appropriate. We also provide advice on Equality Act 2010 compliance and the rules governing pre-employment health enquiries.
Roles and Sectors We Serve
We provide new starter employee health screening for a wide range of roles and sectors. These include safety-critical roles such as HGV drivers, crane operators, forklift operators, and machine
operatives, healthcare and social care workers, where vaccination status and pre-exposure prophylaxis are relevant, food handling and catering roles, construction and site workers, office and
administrative staff, remote workers, and night workers. We can advise on the appropriate level of screening for any role.
Service Options and Pricing
Our new starter employee health questionnaire service is structured into three clear tiers, allowing you to choose the level of support that best suits your organisation’s needs. All tiers include a professionally drafted, HSE-aligned health questionnaire tailored to your sector and role requirements.
| Service Tier | What Is Included | Price |
|---|---|---|
| Tier 1 — Basic Online Questionnaire | Paper or digital health questionnaire, medical screening review, and Pass/Fail certificate issued per employee. No telephone follow-up included. | £40 per employee |
| Tier 2 — Questionnaire + Phone Follow-Up | Everything in Tier 1, plus a telephone follow-up call with the employee where the questionnaire flags a concern. Clearance letter or advisory note issued following the call. | £50 per employee |
| Tier 3 — Escalation to Formal OH Consultation | For employees flagged as requiring formal occupational health review. Includes a full remote OH consultation (telephone or video) with a written management report, fitness opinion, and recommendations. Additional fee on top of Tier 1 or Tier 2. | £225 additional fee |
Volume Discounts
We offer structured discounts for organisations with regular or high-volume new starter intakes. For contracts of 25 to 50 new starters per year, a discount of £5 per questionnaire applies across all tiers. For contracts of 50 or more new starters per year, a discount of £10 per questionnaire applies. To discuss a volume contract, please contact our business enquiries team and we will put together a tailored agreement for your organisation.
Sector Complexity
We understand that not all roles are equal when it comes to health screening. Healthcare workers, food handlers, safety-critical operatives, and regulated professionals all carry additional
considerations that a standard questionnaire may not adequately capture. Our screening process is designed to accommodate this complexity, and our Occupational Health Physician will flag and review any responses that warrant further scrutiny given the nature of the role
24-Hour Turnaround
For straightforward cases, we aim to issue your Pass/Fail certificate or clearance letter within 24 hours of receiving the completed questionnaire. This means your new starter can be cleared and on-boarded without unnecessary delay, keeping your HR process moving efficiently.
Bespoke Questionnaires
Our questionnaires are not one-size-fits-all. They are tailored to HSE standards and to the specific risk profile of the role. Where a role involves manual handling, night working, Hand-Arm Vibration
Syndrome (HAVS) exposure, respiratory hazards, or other occupational risks, the questionnaire will include specific screening questions relevant to those hazards — ensuring you capture the health information that actually matters for the job.
Volume Contracts and Bulk Discounts
For organisations with a steady stream of new starters, our volume contract pricing makes pre placement screening highly cost-effective. Whether you are onboarding a handful of new employees per month or running large seasonal intakes, we can structure an agreement that works for your budget and your HR team’s workflow.

Under the Equality Act 2010, employers are generally restricted from asking health-related questions before a formal job offer has been made. However, once a conditional job offer has been made, new starter employee health questionnaires are entirely permitted and represent standard good practice for most employers. We will advise you on the legally compliant use of health questionnaires as part of our service.
Our Occupational Health Physician will review the disclosure professionally and confidentially. We will advise on whether the condition affects fitness for the role, what adjustments — if any — are recommended, and whether any further assessment is warranted. We do not share clinical detail with your HR team unnecessarily; our report focuses on fitness for work and workplace recommendations rather than clinical diagnosis.
Yes. We offer corporate contracts for businesses with regular or high-volume new starter requirements, including multi-site organisations. Please contact our business enquiries team to discuss a tailored agreement
