What Is a Food Handler Medical Certificate?
A Food Handler Medical Certificate is an official document confirming that an individual working with food is medically fit to do so safely and does not pose a risk to public health through the handling, preparation, or service of food. It is issued by a qualified, GMC-registered medical practitioner following a structured
occupational health assessment. Under Regulation (EC) No 852/2004 on the hygiene of foodstuffs and the Food Safety Act 1990, food business operators are legally responsible for ensuring that food handlers who suffer from — or are carriers of — diseases likely to be transmitted through food are excluded from food-handling activities. A formal medical certificate provides clear, auditable evidence that this duty has been met. At Cherries Medical Clinic, the certificate is issued on the day of appointment by our GMC-registered GP — making it one of the most efficient and cost-effective compliance solutions for food businesses of all
sizes.
Who Needs a Food Handler Medical Certificate?
his certificate is required or strongly recommended for workers in a wide range of onshore food-related environments, including new starters and workers returning after gastrointestinal illness. Typical roles include restaurant and café staff, hotel and hospitality catering, hospital and care home kitchens, school and college catering, construction site catering, food manufacturing and processing, supermarket and retail food counters, food delivery and distribution, events and festival catering, and airport and airline catering (onshore).
Onshore vs. Offshore Food Handler Certificates: This page covers the onshore Food Handler Medical Certificate (£100). If you work on an offshore oil & gas platform, wind installation, or vessel, please see the OEUK Offshore Medical with Food Handlers Certificate (£160), which combines the full OEUK offshore fitness assessment with the food handler certificate in a single appointment.
What Does the Assessment Include?
The assessment is a focused occupational health examination designed to identify any medical conditions — particularly gastrointestinal, dermatological, and infectious disease-related — that may make an individual unsuitable for food-handling work at the time of assessment.
✔ Full medical history review, including gastrointestinal symptoms, skin conditions, or infectious disease history
✔ Assessment for conditions transmissible through food (Salmonella, E. coli, Norovirus, Campylobacter, Hepatitis A, typhoid carrier status)
✔ Examination of hands, skin, and any wounds or skin conditions relevant to food hygiene
✔ Review of current medications and any conditions affecting food safety
✔ Assessment of respiratory conditions affecting food hygiene compliance
✔ Professional opinion on fitness for food-handling duties
✔ Signed, dated Food Handler Medical Certificate issued on the day of appointment
UK Legal Requirements for Food Businesses
UK food law — particularly Regulation (EC) No 852/2004 (retained in UK law post-Brexit) and the Food Safety and Hygiene (England) Regulations 2013 — places a clear duty on food business operators to ensure food handlers are medically fit. Food business operators must ensure that anyone known to be suffering from, or to be a carrier of, a disease likely to be transmitted through food is excluded from food-handling roles. Workers with gastrointestinal symptoms (diarrhoea, vomiting, infected wounds, or skin infections) must be excluded from food-handling work until symptom-free and medically cleared.
Employers should hold documented evidence that they have taken reasonable steps to assess and manage the health of food handlers, and local authority environmental health officers may request evidence of medical fitness during food hygiene inspections. A formal medical certificate from a GMC-registered GP provides robust, auditable evidence of compliance and reduces the legal exposure of the food business operator.
Appointment Requirements
✔ Bring valid photo ID (passport or driving licence)
✔ Bring details of your food-handling role and employer if available
✔ Inform the doctor of any current gastrointestinal symptoms, skin conditions, or recent infections
✔ Bring a list of any current medications
✔ No fasting or special preparation is required for this assessment

While UK law does not mandate that every food handler must hold a specific medical certificate before starting work, the Food Safety Act 1990 and Regulation (EC) No 852/2004 require food business operators to ensure that food handlers are medically fit and do not pose a risk to public health. Many employers — particularly in healthcare catering, hospitality, food manufacturing, and contract catering — require a formal certificate as part of their pre-employment or periodic health surveillance process. Local authority environmental health officers may also request evidence of medical fitness during inspections.
A certificate cannot be issued if the food handler is currently suffering from, or is a known carrier of, a disease likely to be transmitted through food. This includes active gastrointestinal infections (Salmonella, E. coli O157, Campylobacter, Norovirus, or typhoid), active Hepatitis A infection, infected skin wounds or dermatological conditions affecting the hands, or any other condition the examining doctor considers poses a risk to food safety. The doctor will advise on steps required — such as treatment or symptom resolution — before a certificate can be granted.
There is no statutory validity period specified in UK food law. The certificate reflects fitness status at the date of assessment. As a general guide, many food businesses request a new certificate annually or upon return after a significant illness or absence. Where a food handler develops gastrointestinal symptoms or an infectious illness after a certificate has been issued, they must be excluded from food-handling work regardless, and a new assessment is required before return to duties.

No. A food hygiene certificate (e.g. Level 2 Award in Food Safety) is a training qualification demonstrating that a food handler has been trained in food hygiene principles. A Food Handler Medical Certificate is a medical fitness assessment confirming the individual does not carry or suffer from a disease transmissible through food. Both may be required, but they serve different purposes and are issued by different bodies.
