What Is an Ill Health Retirement Report?
If you are considering retirement on the grounds of ill health, or if your employer or pension scheme requires a medical report to process an ill health retirement application, Cherries Medical Clinic can provide an independent, professionally authored Ill Health Retirement Report. Our GMC-registered Occupational Health Physician is experienced in preparing reports across a wide range of public and private sector pension schemes and works with both individual employees and employers or HR teams.
Pension Schemes We Support
We prepare ill health retirement reports for the NHS Pension Scheme (including Tier 1 and Tier 2 assessments), the Teachers’ Pension Scheme, the Local Government Pension Scheme (LGPS), the Civil Service Pension Scheme (Alpha, Classic, and Premium), the Police Pension Scheme, the Firefighters’ Pension Scheme, private occupational pension schemes, and employer-specific HR ill health retirement processes. If your scheme is not listed here, please contact us and we will advise whether we can assist
When Is an Ill Health Retirement Report Needed?
You may need an ill health retirement report if you have a permanent health condition that prevents you from performing your job, if your employer or pension scheme requires independent medical evidence to support your application, or if you have been absent from work for an extended period due to ill health. The report is also commonly needed when your own GP is unable or unwilling to complete the required pension scheme forms, when you need a second medical opinion to strengthen your application, or when an employer or HR manager wishes to facilitate an employee’s ill health retirement compassionately and correctly.
What the Report Covers
The report involves a full review of your relevant medical records and history, an assessment of your current health status and functional capacity, and a professional opinion on whether your condition is permanent and likely to prevent a return to work. Where applicable, a Tier 1 and Tier 2 opinion will be provided, for example for NHS Pension applicants. The report also addresses the prognosis and expected trajectory of the condition, completes any pension scheme forms required, and produces a written report suitable for submission directly to the relevant pension authority.
How to Arrange a Report
Please contact us in the first instance to discuss the pension scheme involved and the specific report requirements. We will confirm the fee and arrange an appropriate consultation — either face to-face at our Slough or Southampton clinic, by video, or as a home visit where this is needed.
Following the consultation and review of medical records, the report and any required forms will be completed and issued within the agreed timeframe.
Pricing
Ill Health Retirement Reports are priced at £300. Pricing may vary depending on the pension scheme, the level of report required, and whether a home or clinic visit is needed. Please contact us before booking and we will confirm the fee and advise on the appropriate report type before you commit to anything.

Yes. If a pension scheme has rejected an initial ill health retirement application, our independent report can be used to support an appeal or to provide additional medical evidence in support of a reconsideration.
Yes. GPs who are unable or unwilling to complete the required pension scheme forms themselves may refer patients to us. Solicitors and HR teams may also instruct us directly.
This depends on the pension scheme and the complexity of the medical history involved. We will give you a realistic timeframe at the point of enquiry and will keep you updated throughout.

Dr Anwar is exceptionally helpful and professional. In a very short period he provided an appointment , met with my family member and provided a supporting letter to bolster an application for exemption from the Life in The UK test.
