✅ Pre-Health Questionnaire   

✅ Examination of vitals (Blood pressure, Heart Rate, Oxygen Saturation, Respiratory Rate)  

✅ Musculoskeletal assessment

✅ Cardiovascular examination (auscultation of the heart valves)

✅ Respiratory Examination (auscultation of the lungs including lung spirometry)

✅Audiology Assessment

✅Urinalysis check (to rule out protein, blood, glucose) 

✅Subject to Employer Request Only:- Drug & Alcohol Testing (separate additional fee) 

✅Subject to Employer Requirement:- Chester Step Test +/- CA-EBS Bosiet Certificate

✅ Photo ID (Work ID, Drivers License or Passport)  

✅ Completion of Pre-Health Questionnaire (To bring printed or physical copy)

✅ Prescription glasses.

✅ Details of any ongoing conditions.

Your Certificate will normally be valid for a maximum period of 2 years from Date of Issue. You will be handed a certificate towards the end of the appointment.
