Step 01

Employee Consent

Begin by discussing the referral with your employee and obtain their informed consent for an Occupational Health (OH) consultation.

Step 02

Referral Form Submission

Complete the Occupational Health Employer Referral Form, clearly outlining your concerns.

Include Supporting Documents:

  • Employee’s sickness absence record (e.g., HR spreadsheet)
  • Job role, working hours, and mental/physical job demands
  • Details of any workplace adjustments (made or considered)

Step 03

Employee Consent Form

Submit a signed Employee Consent Form, which also includes optional authorisation to access medical records from their GP.

Advisory:
Whilst not essential for the initial consultation, recent medical records can enhance the accuracy of the assessment and may be necessary for follow-ups. Please also bare in mind that formal request to access GP medical records can take up to 28 working days as per GDPR legislation from the employee’s primary care practitioner.

Once referral and consent forms are submitted, use the booking link provided to schedule a 1-hour consultation.
